Please have your insurance card and proof of ID at the time of your visit.You may owe us an amount after our bill is processed by your insurance company. Based on your insurance plan and medical benefit, you could be responsible for charges beyond your co-payment paid at the time of the visit for things like your deductible or co-insurance or services not included in your benefit. Instead of sending you a bill, we can automatically charge your balance due to the credit card of your choice. You will be asked to authorize these payments at the time or your visit.

Out of Pocket

If you haven't met your deductible, don't have insurance or don't have a plan we currently accept, we accept cash, credit cards, and debit cards. Sorry, but we do not accept personal checks. If your plan offers out-of-network benefits, we will give you a receipt you can submit to your insurer for reimbursement. You may call to discuss cost of each procedure, diagnostic test and vaccine.
